You can also get a piece of pvc from Grainger and cut it to use as a shim. Cost is about 4.50 for 3'.
Works great.

On the scotty rail mount there are 2 1/4-20 set screws in the bottom, I took them out and put 1 inch ss 1/4 -20 bolts in and tightened works just fine.

These will work fine. All you need to do is use 10/32" bolts instead of the supplied 1/4-20 bolts. The smaller diameter bolts will provide the clearance necessary to fit around the hand rail bar.

The Scotty #245 clamp on mount might be easier. You will need to add a thin shim between the rod holder and side rail, I used a piece of 1/8" thick rubber. Bicyclye tubes are about the perfect thickness.
