I recently did a similar upgrade to my '81 H16 jib system using two swivel cam cleats, Trentec's car, and one cheek block on each side.

Then make sure the swivel cleat it returns through is positioned forward enough on the rail for the outhaul line to miss the swivel cleat that the jib sheet runs through.

Tie it from the inboard hole of one to the inboard hole of the other. This is what returns them to center when you uncleat the outhaul line.
THEN, I saw this set up on a boat at the boat yard and realized that the twin swivel cam cleats just might be overkill as MBounds points out.
